RANITIDINE RECALL REIMBURSEMENT POLICY

Glenmark Pharmaceuticals Inc. 

As of December 17, 2019, Glenmark Pharmaceuticals Inc. (“Glenmark”) voluntarily recalled all unexpired lots of its Ranitidine Tablets, 150 mg and 300 mg.  See recall announcement. 

Consumers who purchased Ranitidine Tablets that are the subject of this recall may be eligible for reimbursement of their out-of-pocket (co-pay) expenses incurred in the purchase of the product. Consumers can call Qualanex at 1-888-504-2012, Monday - Friday, 9:00 am – 5:00 pm EST, and must provide the following information required for verification of their purchase of the impacted product:

• NDC number of the dispensed product, which can be located on the first page of the original invoice, next to the Rx number;
• Pharmacy receipt showing the date of purchase and payment of the out-of-pocket (co-pay) expense.
